*We are providers for all UnitedHealthcare networks, however UnityPointHealth-Meriter Hospital is not in-network for UHC Core and UHC Select plans. If you are in one of these networks and may have labs or services through our providers that are performed at Meriter Hospital, those may not be covered. Please contact your plan ahead of your visit to confirm coverage.
Other Insurance Plans
We also accept other non-network plans and fee-for-service insurance, such as “Choice” plans or “Point of Service (POS)”, as well as self-pay arrangements. Patients under these plans may pay more out-of-pocket to see our providers.
